Nipple and Areola

Nipple & Areola Correction

Inverted nipples, large or uneven areolas and elongated nipples are common and can be corrected with short procedures — usually under local anaesthesia, with discreet scars.

Inverted nipples

An inverted nipple points inwards instead of outwards. It can be present from birth or develop later. Inversion is graded by how easily the nipple can be drawn out:

  1. Grade 1 — the nipple is easily pulled out and stays out for a while.
  2. Grade 2 — the nipple can be pulled out but returns inwards.
  3. Grade 3 — the nipple cannot be drawn out because the ducts are short and tight.

A nipple that becomes inverted later in life, especially on one side, should always be checked with breast imaging first.

How an inverted nipple is corrected

Through a tiny incision at the base of the nipple, the tight fibrous bands are released and the nipple is supported from beneath with stitches so that it stays projected. In grade 1 and some grade 2 cases the milk ducts can be preserved; in grade 3 some ducts often have to be released, which may affect breastfeeding.

Areola reduction

Large or puffy areolas can be reduced by removing a ring of pigmented skin around the edge and closing it with a permanent, purse-string stitch. The scar lies at the natural border between the areola and the breast skin. Uneven areolas can be matched in the same way.

Nipple reduction

Elongated or wide nipples can be shortened or narrowed by removing a small segment of tissue while preserving the central ducts and sensation wherever possible.

Recovery

These procedures usually take thirty to sixty minutes under local anaesthesia and you go home the same day. Light dressings are worn for about a week; most people return to work within a day or two.

Because these procedures are performed under local anaesthesia, they can be completed in a short visit or combined with another breast operation.

Frequently asked questions

Can an inverted nipple come back after correction?

Recurrence is possible, especially in grade 3 inversion, but is reduced by supporting the nipple from beneath during surgery.

Can I breastfeed after inverted nipple correction?

When the ducts are preserved, breastfeeding is often still possible. If ducts must be released, the ability to breastfeed may be reduced.

Can areola reduction be combined with other breast surgery?

Yes. It is often part of a breast lift, reduction or tuberous breast correction.
